Student Registration Requirements

 at W.T. Sampson

 

Registration of your school age children can be accomplished by reporting to the main office of either the elementary for grades P/K-6 or the high school for grades 7-12.

Registration may be completed:  Monday through Friday, 8:00 am to 4:00 pm

Families need to bring the following required documents at the time of registration.  Registration must be done in person at the school. Without the necessary paperwork your child cannot be enrolled.  Every effort will be made for the appropriate placement of children enrolling during the mid-year.

  • One Copy of sponsor's PCS orders assigning sponsor and dependents to Guantanamo Bay
  • Copy of Dependent Entry Approval
  • Student's Birth Certificate or Passport for each child
  • Student's Social Security Card
  • Student's Immunization records
  • Records from Previous School
  • Valid ID Card of Parent

When transferring out of W.T. Sampson Unit School during the school year, the following procedures should be followed:

  • Parents must advise the Registrar at least 10 days before the departure date so that grades and school records can be prepared. 
  • Provide the Registrar with a copy of the orders as soon as available.
  • Advise each teacher (through the counselor and/or office) of the departure date.
  • By the day before the actual withdrawal date, the student should be sure all obligations have been met including turning in materials and paying for anything lost or damaged. On the day of the withdrawal date, the student must report to the Registrar's Office to pick up a Clearance Form. This form must be completed during the day and returned to the Registrar's Office before departing school. This must be accomplished before school records may be released. Parent may pick up hand-carry copies of records after completion of clearance procedures.
